• Return Materials to the Instructional Materials Center

  • This form is NOT for surplussing old, unused materials. Use this form only for materials that are district-adopted and barcoded or for pilot materials that need to be processed.

    Directions for use of this form:

    • BEFORE returning books to IMC, ensure that the books are checked IN in Destiny.
    • BEFORE packing materials, order boxes from IMC to ensure that packed boxes can be handled by Carriers and IMC staff.  Packages that are too heavy or large will be refused.
    • Submit your information. You will be emailed a confirmation page.
    • Pack materials needing repair separately from excess materials.
    • Print the email confirmation and affix a copy of it to each package you send to the IMC for this transfer of materials.
    • Place the packages of materials in your school's mailroom location.
    • If the materials are anywhere other than your mailroom, please complete a warehouse request form. Your office manager can help with that.
