This form is NOT for surplussing old, unused materials. Use this form only for materials that are district-adopted and barcoded or for pilot materials that need to be processed.
Directions for use of this form:
- BEFORE returning books to IMC, ensure that the books are checked IN in Destiny.
- BEFORE packing materials, order boxes from IMC to ensure that packed boxes can be handled by Carriers and IMC staff. Packages that are too heavy or large will be refused.
- Submit your information. You will be emailed a confirmation page.
- Pack materials needing repair separately from excess materials.
- Print the email confirmation and affix a copy of it to each package you send to the IMC for this transfer of materials.
- Place the packages of materials in your school's mailroom location.
- If the materials are anywhere other than your mailroom, please complete a warehouse request form. Your office manager can help with that.
