Request for Reconsideration of Library Materials

Per School Board Policy 2021 and Procedure 2021P, Northshore School District has delegated the responsibility for evaluation and selection of library materials to school-based teacher-librarians. Northshore has established reconsideration procedures to address concerns about library materials. Completion of this form is the first step in those procedures.
Materials Selection:

Per School Board Policy 2021 and Procedure 2021P, Northshore School District has delegated the responsibility for evaluation and selection of library materials to school-based teacher-librarians. Suggestions for materials may originate from students, administrators and teachers and these suggestions are considered by the teacher-librarian, who weighs requests, evaluates materials, and selects materials that fulfill the needs of the current, district-wide and school based instructional programs.

Procedures for Reconsideration:

Completing this form will initaite a request to reconsider materials in a school or district library catalog. The form's contents will be sent to your school's teacher-librarian and principal for review and you will be contacted for further conversation. After completion of the review and conversation, you will be notified of next steps.
