NSD Student Device Summer Opt-Out (Grades 2-11 only)

Families who do NOT want to take home their student's assigned computing device over the summer should complete this form to indicate their preference. Students who opt out and who are returning to Northshore schools in Fall 2026 will be reassigned a device for learning when they return to school. Please complete this form no later than June 9 so that your school can prepare to collect your student's device.
