Panorama Survey Opt Out Form

Completing the opt-out form will preclude the student from participating in the Fall 2026 Panorama survey administration AND the Spring 2027 Panorama survey administration. We kindly ask families who wish to remove students from the Panorama survey process during the 2027-28 Academic Year to complete the form again at the beginning of that academic year.
